Install a dictionary on your computer, such as "euroglot" and immediately look up every word you don't understand. Use firefox, chrome or iron as your internet browser and install the option to spell-check everything you write immediately (this way, you immediately notice when you are making errors, it really helps to learn to write). Watch a lot, a LOT of English and American television series, movies, etc. Once you get a little better, listen to a bunch of British and American talking shows and radio shows that you can get from torrent downloads, shoutcast, all kinds of podcast sites, etc. Read a lot of English books, particularly the clics and quality literature (it has complicated sentence constructions which increase your understanding of the language). Go on holiday to an English speaking country if you can.
